Our Services

Assessments

Caring Connections provides professional expertise in assessing our clients’ health status and their safety in their current living environment. We help determine if there is a need for additional support services, and we refer clients to local community resources that will meet their needs. We provide a professional perspective on family members’ most important concerns:

  • “My aunt seems confused lately—could it be Alzheimer’s or dementia?”
  • “We think it’s time to move our father to assisted living, but he’s resisting.”
  • “I live 500 miles away. How can I be sure the nursing home is treating my mom well?”
  • “I’m considering two different retirement communities—which is the best fit for me?”
  • “Dad is in the hospital, but I’m not sure the doctors are telling me everything I need to know.”
  • “Is my mother at risk of injuring herself in her home?”

With Caring Connections on the case, families will not feel alone when considering the big questions related to aging. We help our clients and their families gather information and access resources so they understand all the options available to them.

Medical Case Management

Caring Connections ensures that the medical needs of our clients are being met. We assist in the coordination of health care services, provide support during doctor visits, review prescribed medications, and offer professional expertise and education for families about their loved ones’ health-related needs.

Support for Families/Caregivers

We understand that it is difficult for family members to constantly be present for their aging loved ones. Long distances and busy schedules make it difficult to attend to every need. Caring Connections brings families peace of mind by visiting our clients regularly, and reporting back to their families. Family members can rest easier, knowing that we are checking on their loved one.   We will:

  • Visit the client at regularly scheduled times
  • Listen and respond to the client’s needs
  • Monitor the client’s ongoing health status
  • Provide emotional support during times of change
  • Provide social interaction for the client and help prevent feelings of isolation

As a result, family members will:

  • Receive regular reports (oral or written) on the client’s overall health and well-being
  • Be informed of any urgent needs that require action by the family
  • Benefit from a “second opinion,” a professional perspective on decisions they make regarding care of their loved one
  • Feel confident that their loved one is in good hands

Transportation

When appropriate, Caring Connections provides assistance to help our clients get to medical appointments and other important locations. We can also arrange appropriate transportation from other service providers when needed (such as scheduling a van for a wheelchair-bound client).

Short-Term Coverage

In cases where ongoing visitation is usually done by family members, we can provide back-up if the family member is unavailable to visit their loved one.

  • Visits while a family member is out of town
  • Emergency coverage for an unexpected illness
  • Be a local presence until out-of-town family can arrive
